Open enrollment for 2025 is April 15 to May 15 for coverage effective July 1, 2025.

- Resident of Utah
- Licensed as an EMS provider
- Provide EMS services for a qualified rural agency
- Respond to 20% of calls over a rolling 12-month period
- For medical and dental insurance, do not have affordable coverage available through an employer, a spouse’s or parent’s employer, or a government plan or program
What happens if I lose eligibility?
If you no longer meet all of the eligibility requirements, your EMS coverage will end on the last day of the month in which any one of the conditions for eligibility no longer applies.
